I know batteries must be removed, what about the covers, do they have to be left off?

If by cover you meant battery compartment cover, you can put it back if you want. Unless you’re sending 50+ phones though you’re only hurting yourself. A battery cover isn’t enough to change the value up any, but with mail, every ounce counts against you.
