These came out of a dish net work DVR, 164.4GB HDD.

Appears that the drive self destructed itself. Probably caused by the read, write head because it was damaged. Aluminum platters shown.

Op: That’s a nasty head crash. Not the worst I’ve seen but close. To get to that point you’re intentionally ignoring read/write errors. And the unbearable high pitch shrieking.
Looks like glass platters.
