What are these and how are these graded? Are they simply Apple CPUs?

Left one is obverse, right one is the same board's reverse.

I had one of these a while back and it was graded as an Apple CPU (even though it didn't come out of an Apple).

Most if not all Apple processors from this era came in that package/form factor.
Dead giveaway is the connector in the bottom right on the reverse where it connects to the motherboard in a "press-in" fashion, instead of using a normal zif socket/slot as was common in all other MB's (thanks for being different, Apple).
You can disregard the memory slot, that was so Apple could charge you 2k for an upgrade later on.
